Interested applicants who wish to apply for hiring are required to submit the following documents to the Human Resource Development Division (HRDD) :

  • Application letter (indicating the position and place of assignment where the vacancy exists);
  • Resume (with character reference);
  • Duly Accomplished Personal Data Sheet (PDS) -2017 revised form with recent passport size (4.5 cm x 3.5 cm) picture with complete and handwritten name tag and signature over printed name.
  • Work Experience Sheet;
  • Copy of Transcript of Records (TOR);
  • Authenticated Copy of Certificate of Eligibility / PRC License;
  • Most recent performance rating / IPCR,IPAR;
  • Copy of Employment Certificates with actual duties and responsibilities;
  • Copy of Training Certificates;
  • Data Privacy Consent form and
  • Background Investigation Waiver
  • Applicants for reappointment (promotion / renewal) are required to submit the following :

  • Application letter (indicating the position and place of assignment where the vacancy exists);
  • Accomplished Personal Data Sheet (PDS) -2017 revised form with recent passport size (4.5 cm x 3.5 cm) picture with complete and handwritten name tag and signature over printed name.
  • Work Experience
  • Applicants must submit one (1) set of documents for every position being applied for.
  • Application for multiple positions is allowed provided that the applicant complies with the required qualification standards and should specifically identify the intended positions.
